For First Year Patients
All Plans below are publicly available first year plans. Adult Plans include: One home environmental assessment, an environmental plan of care, gender & age specific lab panels (blood, stool, urine, and hair requirements vary) at the beginning and end of the plan (labs drawn 2x); up to three follow up nurse visits; two hormone therapist visits with a specialized Family Nurse Practitioner (“FNP”); three specialized Registered Dietitian (“RD”) visits; and an initial Pharmacist (“PharmD”) consultation including prospective product recommendations, if appropriate. Product recommendations are custom per patient (lab results considered) and will be billed separately. Purchased products come with ongoing complimentary PharmD support. MyoCoast Therapy (a dental, airway, sleep, and breathing specialist) is only included in the Child’s Plan.
